Introduction:
A condition in which blood sugar is high, but not high enough to be type 2 diabetes, it might be called as impaired fasting glucose or impaired glucose tolerance. Without intervention, it is likely to become type 2 diabetes within 10 years. Many people with prediabetes have no symptoms. Progression from prediabetes to type 2 diabetes is not inevitable. With lifestyle changes, weight loss and medication, it is possible to bring a blood sugar level back to normal.

- Banaba Leaf (Lagerstroemia speciosa) Extract – Banaba leaf extract helps lower blood glucose in people with type 2 diabetes. It might help the body use insulin more efficiently. In pre-diabetes patient, it improves the production of insulin in the body. It also improves the sensitivity of the body to the effects of insulin. It has anti-diabetic property. Banaba leaf contains corosolic acid (CA), a compound which prevents oxidative stress, inflammation and hypertension. The hypoglycemic effect of CA has been reported by various experiments. The decrease in insulin resistance due to the increase in GLUT4 translocation has been reported as a mechanism of action. Increased glucose uptake in the small intestine has been known as one of the major pathogenic factors of type 2 diabetes mellitus, together with the insulin resistance in peripheral tissues and the impairment of glucose induced insulin secretion from pancreatic beta cells. The extract from the banaba leaf contains polyphenol compounds. It has been reported that polyphenol shows glucose-lowering effects by inhibition of carbohydrate absorption and decreases the uptake of glucose in the small intestine.
- Fenugreek (Trigonella foenum graecum) Extract – Fenugreek extract helps to lower blood glucose in patients with diabetes. It reduces fasting blood glucose levels and improves glucose tolerance. It has anti-diabetic property. Fenugreek oil significantly improved blood glucose levels, glucose intolerance, and insulin sensitivity compared to the diabetic group. The pancreatic islet and less β-cells damage were observed after the administration of fenugreek oil to diabetic rats. Moreover, diabetic rats showed low activities of superoxide dismutase, catalase, glutathione peroxidase, and reduced glutathione content in kidney, which were restored to near normal levels by treatment with fenugreek oil. The increased levels of lipid peroxidation, creatinine, albumin, and urea in diabetic rats decreased significantly in diabetic rats treated with fenugreek oil. Diabetic rats treated with fenugreek oil restored almost a normal architecture of pancreas and kidney. In conclusion, it reveals the efficacy of fenugreek oil in the amelioration of diabetes, hematological status, and renal toxicity which may be attributed to its immunomodulatory activity and insulin stimulation action along with its antioxidant potential.
- Bitter melon (Momordica charantia) Extract – Bitter melon is linked to lowering the body’s blood sugar. Bitter melon has polypeptide-p or p-insulin, an insulin-like hypoglycemic protein that act like insulin, which helps bring glucose into the cells for energy. It has anti-diabetic property. Bitter melon extract helps to decrease blood glucose levels because it contains charantin, polypeptide-P insulin, and lectins, which are useful substances to decrease blood glucose level. Hyperglycemia indication in diabetic alloxan-induced mice occurred due to the alloxan toxic effects that damaged the insulin receptor which followed by pancreatic β-cells destruction. As a damage result of insulin receptor and pancreatic β-cell, the insulin cannot be produced normally. Therefore, the blood glucose cannot be retrieved and converted into energy, so that glucose levels in the blood is increased. The natural toxic of alloxan triggers free radicals’ formation that can damage cell membranes by increased lipid peroxidation that causes disruption of essential ion transport from and within the cell, which causing the cell death. The effects of free radicals by alloxan induced can cause oxidative stress in pancreatic β-cells and lead disturbance to the oxidation phosphorylation process. This mechanism increases the free radical’s production. Furthermore, antioxidants in the body is no longer able to change the reactive oxygen (O) into a neutral compound O2. Alloxan will react with two SH-groups of the glucokinase enzyme dimer and inactivate the insulin enzyme. Therefore, it disturbs the insulin secretion and damage the β-cell, which leads to diabetic risk. The pancreas treated with bitter melon extract has a significant increase in the size, number, and regeneration of Langerhans islet cells. The bitter melon extract can suppress the blood glucose levels, prevent the intestine from absorbing the glucose, and improves pancreatic β-cells to produce insulin.
- Grape seed Extract – Grape seed extract may help to lower blood sugar and glucose levels. It has antioxidant property. Grape seed extract reduces the post parandial hyperglycemia by attenuating the absorption of glucose. In a normal, metabolic condition, this is achieved by the enzyme’s alpha-glucosidase, alpha-amylase and pancreatic lipase. The ethyl acetate fraction of Muscadine grapes (Vitis roundifolia) has anti-glucosidase and pancreatic lipase activity which could be attributed to the phenolics such as quercetin and ellagic acid, present in the seeds also the aqueous extract of grape seeds are potent alpha glucosidase and alpha amylase inhibitors. The oligomers of grape seed procyanidin extract phosphorylates the insulin receptor and thus activates to increase cellular glucose uptake in 3T3L1 cells. Further it was confirmed that procyanidins from the grapes increases the cellular glucose uptake in insulin sensitive L6E9 and 3T3L1 cell lines by insulin-mimetic property.
- Ashwagandha Extract- Ashwagandha extract influences the body’s production of insulin and insulin sensitivity. It stimulates the secretion of insulin in bloodstream thus improving the breakdown of sugars in the blood. This ensures that less harmful sugars are being stored on the body. Thus, the blood sugar levels remain low. It has anti-diabetic property. An ayurvedic herbal formulation (Tarsina) containing Withania somnifera (WS) as one of the ingredients has been found to attenuate streptozotocin-induced hyperglycemia and pancreatic islet superoxide dismutase activity in type 1 diabetic rats.

Curcumin and its analogues have a mechanism of action similar to that of thiazolidinedione (TZD), an anti-diabetic drug, through peroxisome proliferator activated receptor-(PPAR) activation. Thus, curcumin may be effective in the regulation of glycaemia and lipidaemia factors that play a key role in type 2 diabetes mellitus (T2DM) treatment.
- Magnesium – Magnesium helps regulate blood sugar levels. Intracellular Mg plays a key role in regulating insulin action and insulin-mediated glucose uptake. Reduced intracellular Mg concentrations result in a defective receptor tyrosine-kinase activity, post receptorial impairment in insulin action and worsening of insulin resistance in diabetic patients. A low Mg intake and an increased Mg urinary loss appear the most important mechanisms that may favor Mg depletion in patients with type 2 diabetes. Low dietary Mg intake has been related to the development of type 2 diabetes and metabolic syndrome.
