Introduction:

Polycystic ovary syndrome (PCOS) is a condition that affects a woman’s hormone levels. Women with PCOS produce higher-than-normal amounts of male hormones. Birth control pills and diabetes drugs can help fix the hormone imbalance and improve symptoms. Polycystic ovary syndrome (PCOS) is a hormonal disorder common among women of reproductive age. Women with PCOS may have infrequent or prolonged menstrual periods or excess male hormone (androgen) levels. The ovaries may develop numerous small collections of fluid (follicles) and fail to regularly release eggs. Women with PCOS produce higher-than-normal amounts of male hormones. This hormone imbalance causes them to skip menstrual periods and makes it harder for them to get pregnant. PCOS also causes hair growth on the face and body, and baldness. And it can contribute to long-term health problems like diabetes and heart disease.

Problems associated with PCOS: The symptoms which occurs due to PCOS are:

  • Irregular periods- A lack of ovulation prevents the uterine lining from shedding every month. Some women with PCOS get fewer than eight periods a year.
  • Heavy bleeding-The uterine lining builds up for a longer period, so the periods you do get can be heavier than normal.
  • Hair growth- More than 70% of women with this condition grow hair on their face and body — including on their back, belly, and chest. Excess hair growth is called hirsutism.
  • Acne-Male hormones can make the skin oilier than usual and cause breakouts on areas like the face, chest, and upper back.
  • Weight gain- Up to 80% of women with PCOS are overweight or obese.

  • Gymnema sylvestre Extract (50 % Gymnemic acid) – Gymnema is well indicated for PCOS due to its insulin modulating activity, triglyceride lowering effects, and its ability to inhibit absorption of glucose in the intestine, thereby lowering the glycemic response. It has anti-diabetic property, and it treats PCOS by improving insulin level. Immunomodulatory activities of gymnema sylvestre extract are determined by hemagglutination antibody (HA) titer and delayed-type hypersensitivity (DTH) tests for determining specific and non-specific immune responses. Flow cytometric techniques were used to estimate B lymphocytes (CD3 and CD19) and Th2 cytokines (IL-2, IFN-γ and IL-4).The response produced by oral administration of methanolic extract of Gymnema sylvestre elicited significant reduction in a dose-related manner in the primary and secondary antibody and DTH responses, with maximum reduction observed at 200 mg/kg-body wt. It also demonstrates that G. sylvestre exerts immunosuppressive effects on the components of the immune system of mice, and points to its significant immunomodulatory potential.
  • Fenugreek Extract – Fenugreek seed extract helps increase insulin sensitising activity and peripheral utilisation of insulin, correcting increased androgen hormones. It also helps to reduce hypercholesterolemia. Fenugreek seed extract also cause significant reduction in ovary volume. It has anti-diabetic property, and it treats PCOS by improving insulin level. Antibody production to T-dependent antigen sheep red blood cells (SRBC) requires co-operation of T and B-lymphocytes and macrophages. The high values of haemagglutinating antibody titer obtained in case of methanolic extract of fenugreek whole plant have indicated that immuno stimulation was achieved through humoral immunity. The methanolic extract of fenugreek whole plant exerts capability to influence the role of immunoglobulins results activation of pre-B cells and/or dendritic cells results in activation of antibodies which give the higher agglutination titer against SRBC’s antigens. Increase in rosette formation and lymphocyte formation in T-cell population test indicate effect of methanolic extract of fenugreek whole plant on cell mediated immunity. Methanolic extract of fenugreek showed dose dependant immunomodulatory activity. Methanolic extract of fenugreek may activate the CD4+ and CD8+ cells which influence T-cell mechanism results increase in T-cell immune response significantly.
  • Vitex agnus-castus Extract – Vitex agnus castus (Verbenaceae), commonly referred to as chaste tree or chasteberry, is an aromatic flowering member of mint family. Several potential mechanisms of action of Vintex agnus castus includes inhibition of prolactin secretion, dopaminergic and estrogenic effects. It shows effect on hormonal health in treatment of PCOS. Casticin exhibits a significant inhibitory effect on monocyte oxidative burst in a dose dependent manner. It was found to have a significant suppressive effect on the chemotaxic action at higher concentrations on fMLP (10-8 M) stimulated neutrophils. It also showed a potent suppressive effect on phytohemagglutinin (PHA) stimulated T-cell. PHA is a mitogen for T-lymphocytes, which binds to N-acetylgalactosamine glycoproteins, expresses on the surface of T-cells, and then activates the cells for proliferation. Thus, T-cells are major proliferating cells in peripheral blood mononuclear cells (PBMC) activated with PHA. This PHA proliferation activity was completely suppressed with the addition of casticin at various concentrations. Activation of T-lymphocytes is a complex process. It plays a crucial role in the initiation and regulation of the immune response. It is characterized by secretion of the lymphokine interleukin-2 (IL-2) and by expression of the membrane receptor for IL-2 on T-cells. Stimulation of this receptor by the secreted IL-2 in turn leads to T-cell proliferation. Inhibition of T-cell activation, therefore, provides a powerful approach for immunosuppressive treatment.
  • Ashwagandha (5% withaniolide)- Balances the level of cortisol, which could improve stress and symptoms of PCOS. It also helps to balance hormone levels and thyroid function, support adrenal function, normalizes cortisol levels and maintain insulin levels in the body. It helps to reduce stress in treatment of PCOS. Extract of W.somnifera shows significant increase the cell mediated immunity (CMI) in normal mice. The extact enhances the level of interferon gamma (IFN-ɣ), interlukin-2 (IL-2) and granulocyte macrophage colony stimulating factor (GM-CSF) in mice. Ashwagandha enhances nitric oxide synthatase activity of the macrophages, which in turn increases the microbial killing power of these immune cells. It activates and mobilises macrophages for rendering increased phagocytic activity, potentiates activity of lysosomal enzymes and acts as an anti-stress molecule and anti-inflammatory agent in mice. Roots extract of W.somnifera induces the helper T-lymphocyte (Th1) polarised cell mediated immune response in BALB/c mice .
  • Cinnamon bark Extract- Cinnamon bark extract improves menstrual cyclicity and may be an effective treatment option for some women with PCOS. The cinnamon bark extract contributes to improvement in insulin sensitivity in PCOS. It has anti-diabetic property, and it treats PCOS by improving insulin level. Oral administration of cinnamon bark water extract (CWE) was able to decrease systemic levels of IFN-γ, but not IL-4 and IL-2, secreted by activated T cells. In vitro, CWE inhibited anti-CD3 Ab-stimulated IFN-γ and IL-4 in splenocytes at the mRNA and secreted protein levels. However, it reduced IL-2 mRNA expression but enhanced IL-2 protein secretion at the cellular level, which occurred via its effect on non-T cells. Due to its ability to increase IL-2, CWE decreased cell death. CWE also inhibited anti-CD3-induced p38, JNK, ERK1/2, and STAT4 activation. These observations provided evidence that a CWE can alter inflammatory responses in T cells without changes in IL-2 production.
  • Asparagus racemosus Extract- Asparagus racemosus Extract helps to reduce the stress caused by PCOS. In vitro produced steroidal saponins of Asparagus racemosu (AR) had similar immunomodulatory properties to natural plant root extract. The in vitro produced shataverins stimulated immune cell proliferation and IgG secretion in a dose dependent manner. Many herbal products manifest their effect on immune system by affecting the lymphocyte differentiation pattern and can alter the cytokine profile or mimic the effects of cytokines. Interleukin-6 is a pleotrophic cytokine produced by cells such as macrophages, dendritic cells, and B-cells and therefore can influence differentiation of various immune cells. It has both pro-and anti-immflamatory functions. Wild type AR root extract (WRE) and suspension cultures (SCs) were found to have inhibitory effect on IL-6 expression. IL-6 has two independent mechanisms to alter Th1/Th2 differentiation. Firstly, IL-6 stimulates production of IL-4 by naive CD4 (+) T cells and helps in their differentiation into effector Th2 cells. Secondly, IL-6 inhibits Th1 differentiation by up regulating suppressor of cytokine signaling-1 expression to interfere with interferon gamma signaling and the development of Th1 cells. Since both WRE and SCs had an inhibitory effect on IL-6, these extracts do not drive Th2 differentiation. WRE and SCs stimulates expression of IL-12, which is a potent Th1 stimulator. Hence the extracts have strong modulatory effects on Th1/Th2 cytokine profile. It is reported to restore lymphocyte and neutrophil counts. It modulates macrophage activation and delays tumour development in experimental animals. Immunomodulatory effects of crude WRE extracts on tumor necrosis factor-alpha secretion and phagocytosis is also reported. It also acts as an immunoadjuvent in animal and has strong impact on systemic Th1/Th2 immunity. The immunomodulatory properties of in vitro produced saponins have been described by evaluating IgG production, cell viability and proliferation, cytokine production using unstimulated and stimulated peripheral blood lymphocyte (PBLs).
